This visit includes evaluation and medical management of hormone-related symptoms such as fatigue, mood changes, brain fog, sleep issues, or low libido. Your provider will review your history, symptoms, and labs and discuss appropriate non-pellet hormone therapy options if clinically indicated. Laboratory testing will be required and are included in the cost of this visit. Medications may be ordered and will require additional cost.

This visit is for established patients currently receiving hormone replacement therapy. During this appointment, your provider will review symptoms, response to treatment, side effects, and any updated laboratory results. Hormone therapy may be adjusted as clinically appropriate, and ongoing education and monitoring will be provided to ensure safe and effective treatment. This visit does not include hormone pellet insertion. Additional labs may be required and not included in the cost of this visit.

The initial consultation is a comprehensive medical evaluation focused on weight history, metabolic health, hormonal influences, lifestyle factors, prior weight loss attempts, and current health goals. This visit includes a detailed review of medical history, medications, and contraindications, baseline vital signs and body composition when applicable, and a discussion of treatment options including medication selection, dosing strategy, nutrition guidance, and expectations. Appropriate laboratory testing may be ordered to ensure medical safety and optimize outcomes.

Wellness & Medical Optimization Consultation
This comprehensive consultation with Crystal Johns, FNP-C is designed to evaluate your overall health, symptoms, and goals related to hormone balance, medical weight management, peptides, wellness injections, and nutritional support. Your visit may include a detailed medical history review, discussion of symptoms, review or ordering of laboratory testing, and development of a personalized treatment plan when clinically appropriate. This visit is educational and evaluative; treatment initiation may occur at the discretion of the provider.

This follow-up visit with Crystal Johns, FNP-C is designed to review progress, symptoms, laboratory results, and response to your current wellness or medical treatment plan. This appointment may include medication adjustments, continuation of therapy, review of labs, and refinement of recommendations related to hormone optimization, medical weight management, peptides, wellness injections, or supplements, when clinically appropriate.
